  Work Permit:

 

 

 

1. Download and complete Work Permit Form. 

 

2. Print form,  No form will be considered valid without a Parent/guardian signature.

 

Proof of Age: Attach a copy of a birth certificate, passport, driver's license, etc.

 

3. Attach the signed forms to an email and send it to tney@northernyork.org & jheintzelman@northernyork.org

or have your student bring the signed form to the School Counseling Office.
