  Work Permit:

 

1. Download and complete Work Permit Form. 

 

2. Print form and sign. No form will be considered valid without your signature.

 

3. Attach the signed form to an email and send it to tney@northernyork.org & jheintzelman@northernyork.org

or have your student bring the signed form to the School Counseling Office.
